Five Horizons Health Services provides comprehensive sexual health education to the community that offers a holistic look at an individual’s health through topics including HIV, STIs, physical health, behavioral health, and substance use. Our team is available to host intervention groups or speaking engagements on-site at your location.

Five Horizons Health Services provides supportive services to individuals living with HIV/AIDS. These services include case management, housing assistance, free transportation, food vouchers, and referrals to other supportive services such as behavioral health or workforce readiness. 

• Case Management for individuals living with HIV 
• Food Assistance, Housing Assistance, and Behavioral Health Services Referrals 

Food Pantry- Clients are able to receive one (1) food box per month (canned/pantry items, refrigerator/freezer items).

Perinatal Services- as part of the HOPE Program, clients who are pregnant or within 12 months postpartum receive intensive case management services designed to increase positive health outcomes and validate their experiences during the perinatal period.
